Payment for an office visit (copay, deductible, coinsurance, etc) is to be paid prior to seeing the medical provider.  A co-payment is determined by your insurance contract and and cannot be waived by the provider.  Methods of payment accepted by Lighthouse Pediatrics is as follow:  Cash, Debit Card or Credit Card (Master, Visa, American Express  or Discover credit card)

Insurance Coverage:  Prior to each visit, the patient's insurance eligibility is verified by this office with the insurance vendor. If the insurance policy is not active, Dr Eng is not the PCP or Dr Eng does not participate in your insurance plan, full payment is required at the time of the visit. Insurance eligibility is not a guarantee of payment. Certain plans have restriction for certain services: i.e. vision, hearing test, immunization, laboratory vendor and well child care examination. All cardholders should be familiar with their insurance plan benefits. Any question in regard to your insurance policy or covered services, please contact your insurance company.

Administrative Fees:  

  • Payment (copay, deductible, and coinsurance) not received on day of office visit will be assessed an additional administrative fee of $10.
  • A fee of $20.00 will be charge for any sick visits appointment canceled less than 24 hour notice or a NO SHOW for the appointment.
  • WELL VISIT appointments not canceled > 24 hrs prior to the appointment time or fail to show up for a WELL VISIT appointment will be charge $30 administrative fee.
  • A fee of $35 will be charged for Returned check from the bank (insufficient fund, closed account, etc.).
  • A fee of $5.00 for:  School / camp form, working paper, EIP form and proof of address letter.  Detailed Form: FMLA, college, 504 accommadtion form..etc, $10. fee.
  • Additonal cost of $1.00 - postage to mail out form
  • Note: All school form must be paid prior to being completed by the provider. It takes approximately 5 working days to get school form or letter completed. Due to HIPAA regulation, we can no longer fax medical documents to school, daycare, or camp facility
